ClientTrail: Case Management for Community/Health Services

This Case Management for Community/Health is builtbased on Software as a Service model where the tenant can just subscribe to the application. It can also be installed on the premises as well. The future functionality will be integrated directly to a funding body such as the Department of Community Services or any other institution as a web service. Currently, we are trialling for almost a year now with one of the biggest Not For Profit organisations in Australia an one of their services.
All community and health services strive to help and support those who need it most. That said, customers come in all shapes and sizes. A small domestic health service bears little resemblance to wider financial and gambling services, yet each requires a powerful application to manage and track clients, cases, activities, funding, and outcomes. We understand this.
Transforming the community/health mission and the impact by integrating users, business process and technology is our main goal. We leverage our technology platform, experience in the field and comprehensive services to support the efforts of various community service organisations, including: case managers, team leader, counsellors, admin staff and volunteers. Whatever your role, ClientTrail will support your efforts and bring up improved service coordination and collaboration among your peers and other providers.

Purpose:
1. More for less
2. To propel Not For Proft organisation to meet increasing demands of funders, clients, regulators/government, partners and providers.
3. To transform Not For Profit organisation and maximise their impact by centralising information and simplifying organisation processes related to clients, resources, activities and outcome.
Users:
1. Community/Health Counsellor/volunteer who dealing with the client day to day
2. Team Leader who manage the counsellor
3. Community/Health Manager who manage the data and report back to funding body
4. Community/Health Researcher/Policy Maker. The data that is collected can be used as agregat data for research and set policy
Data: Data collection is based on the report that funding body provide (ei. Commonwealth Financial Counselling, Responsibility Gambling Fund etc). It collects the case, customer and their activity.
This application is a web based application.

Sorry. I can't really see how this is an application that complies with the rules of the contest. It looks more like a company wanting to get some publicity.
